Every rung here is one month, bought once, and the dose is what changes. Citizen Meds says so in capitals on the product page: “THIS PRODUCT FOLLOWS A TITRATION SCHEDULE. ALL PATIENTS MUST START WITH ‘MONTH 1’ DOSE. AFTER MONTH 1, THE DOSAGE PER INJECTION INCREASES GRADUALLY IN MONTH 2, 3, AND 4. LASTLY THE MAINTENAINCE DOSE CAN BE ONGOING FOR UP TO 8 MONTHS DEPENDING ON YOUR GOALS”, spelling as printed. There is an optional subscription at roughly a fifth off each figure — $100, $196, $236, $276, $316 — and it does not hold a price either: it escalates with the dose by default.

The tirzepatide ladder runs six months from $145 to $545, one month at a time. The optional subscription prices are $116, $236, $276, $316, $356 and $436 — and each is billed “every 23 days”, not monthly.
$125 for the first month of semaglutide, $145 for the first month of tirzepatide. Both are one-time purchases and Citizen Meds is unusually plain about it: “– one-time purchase”, “– one-month supply”, “– no subscription necessary”. No membership, no consultation fee — its consent page says the fees “cover the cost of using the Platform, receiving a Physician Review, and any Citizen Meds products prescribed by the Physician.”
Then the dose goes up and the price goes with it. Semaglutide runs $125, $245, $295, $345 and $395 at maintenance. Tirzepatide runs $145, $295, $345, $395, $445 and $545. Six months of tirzepatide bought this way is $2,170, and the last month costs almost four times the first.
There is an optional subscription beside every product, about a fifth cheaper, and its billing period is the strangest on this roster: “every 23 days”. Not a month, not four weeks — twenty-three days, which is nearly sixteen charges a year rather than twelve. No page on the site explains it.
The subscription does not lock your price either. From the FAQ, under Subscription Info: “Patients enrolled in a subscription program will automatically have their dosage increased every month unless they email support at info@citizenmeds.com to request otherwise. Please note that pricing will also increase as dosage increases.” So a $116 tirzepatide subscription walks itself up the same ladder to $436, every twenty-three days, unless you write in and say stop.
Two prices exist for the same first month, on the same site. The homepage and the tirzepatide product page say $145; the ingredient list on all five homekit pages says “– Tirzepatide 2.5mg w/ Glycine and B12 x 4 weeks (First Month Dose) – $185”, and those pages' own headers read $185. The semaglutide split is the same shape, $125 against $135. The checkout table sells at $145 and $125, so those are the figures published here.
Shipping isn't published anywhere on the site, and neither is what the discount code does. Both live past an Add to cart button, which is a boundary this site does not cross.
