No dose table. The semaglutide page prints “Starting at $89/mo*” with the footnote “*With a Starter Kit Plan”, and the FAQ says a higher dose means “your prescription and membership will be adjusted accordingly,” without saying by how much. The comparison pages cap semaglutide at 2.4 mg a week.

The last two columns are our arithmetic, not MaxLife's: the plan's charge against the same number of months bought one at a time at MaxLife's month-to-month price. A prepay only saves that much if you stay on the drug for the whole term.
Fees on top of the medication
Membership. None. The medication price is the whole monthly charge.
Consultation. No separate consultation fee is disclosed.
Billing, canceling and refunds
Subscriptions renew automatically at the chosen billing period, or monthly if none is set, at the then-current rate.
To stop a renewal, email a cancellation at least 30 days before the date you want it to end.
Subscription payments and medication costs aren't refundable, shipped orders can't be canceled, and leaving a commitment plan early can trigger an early termination fee.
Patients found ineligible are refunded what they paid.
