What it is
The Center for Medical Weight Loss, or CMWL, is the online arm of a physician-led weight-loss network that began as brick-and-mortar clinics in 2006. The company behind it is BE-THIN, Inc., which does business as CMWL from Tarrytown, New York, and its chief medical officer is Kevin Jovanovic, MD. Care now runs through a five-minute quiz, a review by a board-certified physician in your state, and home delivery.
It sells compounded semaglutide and tirzepatide, some made with B vitamins or glycine to ease stomach side effects, plus lower-dose microdosing plans for maintenance. Each plan bundles medication, clinical care, unlimited physician messaging and shipping, with no separate membership, and CMWL says it ships to all 50 states.
The current offer is “Get Every 3rd Month Free. Forever.” for members who keep going.
What it actually costs
Advertised$99
Actually$149/mo
Center for Medical Weight Loss advertises $99. A month costs $149 because the advertised rate is the buy 2 Get 1 Free (3 months).
The gap
They advertise $99 a month. That is the buy 2 Get 1 Free (3 months), $298 on the card today. With no commitment it is $149 a month.
Semaglutide is $149 billed monthly, $298 up front on Buy 2 Get 1 Free ($99 a month), or $894 once a year on Buy 6 Get 6 Free ($74 a month). Tirzepatide runs $239 monthly, $478 per three-month cycle ($159 a month) or $1,434 a year ($119 a month). Every dose costs the same.
The home page advertises the three-month rates, $99 and $159. The FAQ adds something the plan cards leave out: three-month options, which ship 12 weeks of medication, are billed every 10 weeks so you don't run out. Your next charge comes sooner than “Billed every 3 months” suggests.
Microdosing for maintenance costs $83 a month for semaglutide and from $116 for tirzepatide, with a three-month minimum.
CMWL takes no money until a physician signs off. Canceling on the same day as your telemedicine visit refunds the order minus $50. Otherwise the FAQ allows refunds within 30 days only if you're found ineligible, can't complete the steps needed to ship, or accept an alternative. Every plan is an auto-renewing subscription.
Who it's wrong for
Reading “Billed every 3 months” as a quarterly bill? The FAQ says three-month supplies are charged every 10 weeks.
Which pharmacy fills the order? CMWL only says U.S.-licensed compounding pharmacies.
Changing your mind after the visit? Even a same-day cancellation keeps $50, and later requests fall under a short list of refund reasons.
Not for you if $239 a month is too much for tirzepatide without prepaying.
Want a live video visit to start? CMWL begins with a quiz and a physician's review, then messaging.
