The short version

In its favor

  • $99 buys one month of compounded semaglutide, no commitment
  • No membership fee on top
  • Same price at every dose
  • Names 4 pharmacies, including RedRock Compounding Pharmacy (St George, UT) and Precision Compounding Pharmacy

Against it

  • The advertised $89/mo is a 12-month prepay, $1,068 at once
  • Consultation fee disclosed, never priced

What it is

Lttl (pronounced “little”) is a telehealth brand selling injectable compounded semaglutide and tirzepatide, a lower-dose “Microdose” version of both, and a needle-free semaglutide tablet. A longevity menu of NAD+, sermorelin and similar injections sits alongside. You take a two-minute quiz, see a plan, check out, and an independent U.S.-licensed clinician reviews the assessment before anything ships.

The company behind it is The Commerce Group Holdings, LLC, and no street address for it is published. Lttl doesn't make or dispense medication. Its terms list RedRock, Health Warehouse, Precision Compounding and Triad Rx as partner pharmacies, and say your prescription may move among them “for fulfillment, availability, pricing, or operational reasons.”

Service covers all 50 states plus D.C. for adults 18 and over, though not every service is offered everywhere. Lttl doesn't bill insurance; HSA and FSA funds often work, depending on the plan administrator.

What it actually costs

Lttl advertises $89. A month costs $99 because the advertised rate is the 12-month supply.

The gap

They advertise $89 a month. That is the 12-month supply, $1,068 on the card today. With no commitment it is $99 a month.

Lttl prices by drug
DrugMonth to monthLowest rate on a longer plan
Compounded semaglutide$99$89 /mo, 12 months
Compounded tirzepatide$149$139 /mo, 12 months
Oral semaglutide$179$139 /mo, 12 months

Semaglutide injections cost $99 on the monthly auto-refill, which you can cancel anytime. Commit longer and the rate drops: $96 a month billed $288 quarterly, $93 billed $558 every six months, or $89 billed $1,068 a year. Tirzepatide runs $149 monthly, then $146 ($438 a quarter), $143 ($858 per half year) and $139 ($1,668 a year).

Each card also shows a crossed-out comparison price, $249 for semaglutide and $339 for tirzepatide, and a “Save” figure measured against it over the whole term. A banner on every page promises “Up to $140 off your first month” with a code applied automatically. What that does to a specific first charge appears only at checkout.

The Microdose line has its own prices. Low-dose injections of either drug open at $119, then renew at $149 a month, or cost $129, $119 and $109 a month on 3, 6 and 12 months. The semaglutide dissolving tablet is $149 for the first month and $179 after, dropping to $159, $149 and $139 on the longer terms.

No membership fee is listed, and dose doesn't change the price. The pricing policy does talk about paying for a “consultation”, and warns that consultation fees may not come back if a provider declines you, without ever putting a number on one.

Refunds are narrow. The terms promise a full refund only for a medical disqualification or a billing error, and once the intake is submitted the order counts as processed and non-refundable. Any refund request has to go in writing to Lttl's support email within 28 days of the charge.

Who it's wrong for

Took “Plans start at $89/mo” at face value? That's the 52-week semaglutide supply, $1,068 charged for the year. Paying monthly costs $99.

Care which compounder fills your vial? Four partners are listed, and Lttl can shift your prescription among them.

Wondering what the consultation costs? Lttl says you pay for one and that it's generally non-refundable, but never prints the amount.

Once the intake's in, money back is off the table except for a medical disqualification or a billing error; the order already counts as processed.

Counting on phone help at night or on weekends? The phone line and care chat run Monday through Friday, 8am to 8pm Eastern, despite the “Unlimited 24/7 support” line on the home page.
