Rx Peps Direct review

GLP-1 telehealth · Prices read September 2026

Our verdict

Rx Peps Direct publishes no single monthly price; compounded semaglutide and tirzepatide are priced per vial, by dose. Month one's vial is $25 for semaglutide or $45 for tirzepatide, plus a $39 visit fee and $15 shipping on each order, and later months cost more as the dose rises, to $108.64 and $247.50 for the month-six vials.

What it is

Rx Peps Direct is a peptide prescription service run by Peptide RX LLC and founded in 2026 by Marcus Hansen. It charges a flat $39 per order for an asynchronous visit: you pick up to three products, fill out a health screening, and a nurse practitioner or physician assistant licensed in your state reviews it, usually within a day. The GLP-1s sit in a catalog of more than 70 compounded formulations.

It doesn't sell the medication itself. After approval, Optimal Balance Pharmacy, a 503A compounding pharmacy in Houston, texts you a payment link, bills its own price and ships FedEx overnight in a cooled case. Every semaglutide and tirzepatide vial carries B12 by default, or glycine with B6 for people who react to B12, and it arrives already mixed.

Coverage is 29 jurisdictions, the District of Columbia among them, and Dr. Jonathan Snipes is medical director. A live 30-minute video visit with him is optional at $149.99, with the $39 fee folded in.

Rx Peps Direct's own charge is $39 per order, covering the provider review and the script for up to three different products. Nothing renews on its own, so a monthly refill is a new order carrying a new $39 fee.

There's no single monthly price. The drug is billed by the pharmacy per vial, and each vial covers 30 days at one dose. Semaglutide starts at $25 for a 1.2 mg month-one vial, then $30, $42.68, $65.96 and $79.54, reaching $108.64 for the month-six maintenance vial. Tirzepatide goes $45, $90, $127.50, $165, $210 and $247.50 over the same six steps. The pharmacy adds $15 for FedEx on each order.

So month one of semaglutide means a $25 vial, the $39 visit and $15 shipping, and tirzepatide's month one swaps in a $45 vial. Each later month costs more as the dose climbs, with the same $39 and $15 on every order. Established patients can buy bigger refill vials, up to $291 for 30 mg of semaglutide and $675 for 180 mg of tirzepatide.

The glycine versions start higher because their smallest vial holds more: $175 for 10 mg of semaglutide and $225 for 60 mg of tirzepatide. Any vial after the starter needs proof that you're under active care for the drug, such as a current prescription or a pharmacy label, unless you're already a patient here.

The $39 is refundable only while your intake sits unreviewed. After that it's kept even when the request is denied, and refunds on medication are up to the pharmacy.

Who it's wrong for

Want a single bill each month? The visit fee goes to Rx Peps Direct, while the vial and shipping go to Optimal Balance Pharmacy through a separate payment link.

Live outside its 29 jurisdictions? Then it can't prescribe to you, and Georgia, Missouri, Ohio and Oklahoma are reachable by the pharmacy but not yet by its prescribers.

Not for you if B12 disagrees with you and price matters: the glycine semaglutide vial starts at $175, against $25 for the B12 starter.

Looking for one fixed monthly price? There isn't one here; the $25 semaglutide vial covers month one only, and by month six the vial alone is $108.64.

Brand Wegovy or Zepbound isn't on the menu; Optimal Balance compounds everything it fills.

  2. The prices, from the product pages' vial selectors and dose charts. /peptides/weight-loss/semaglutide-b12: “Month 1 · 1.2mg 0.6mg/mL · 0.30mg/wk $25 Starter” and “Full dose ladder (pharmacy passthrough at OBP wholesale, no markup): 1.2mg vial $25 · 2.4mg $30 · 4.4mg $42.68 · 6.8mg $65.96 · 8.2mg $79.54 · 11.2mg $108.64 · 12.5mg $121.25 · 15mg $145.50 · 20mg $194 · 30mg $291.” /peptides/weight-loss/tirzepatide-b12: “Month 1 · 12mg 6mg/mL · 3.0mg/wk $45 Starter” and “Full dose ladder (pharmacy passthrough at OBP wholesale, no markup): 12mg vial $45 · 24mg $90 · 34mg $127.50 · 44mg $165 · 56mg $210 · 66mg $247.50 · 30mg/3mL $112.50 · 52.5mg $196.88 · 60mg $225 · 180mg $675.” Both charts say “Concentration steps up monthly; each vial is a 30-day supply at the indicated dose.” and “All prices are pharmacy passthrough at Optimal Balance Pharmacy wholesale, no markup. Add the flat $39 medical visit fee on your first order.” Home page cards: “Semaglutide/B12 From $25/1.2mg”, “Semaglutide/B6 + Glycine From $175/10mg”, “Tirzepatide/B12 From $45/12mg” and “Tirzepatide/B6 + Glycine From $225/60mg”. The microdose products use the same B12 vials: “Tirzepatide Microdose and Semaglutide Microdose, which are the identical Tirzepatide/B12 and Semaglutide/B12 vials drawn at a smaller dose”.
  3. The visit fee and shipping, from /pricing: “We charge one flat $39 fee per order. That is the medical visit. Your peptides are billed separately by Optimal Balance Pharmacy at wholesale, with no markup from us. No subscriptions. No tiers.” and “Typical pharmacy bill: $25 to $200 depending on what you order, plus a $15 flat FedEx overnight shipping fee charged by the pharmacy.” The FAQ: “Each refill is a new $39 visit, and that single visit fee still covers up to 3 different peptides on the same refill order. There is no auto-refill and no pre-paid plan; you order only when you need product.” The dose charts' “on your first order” wording is narrower than the FAQ's per-refill rule; the FAQ is the one that describes refills.
  4. After the starter vial: “Required for any Semaglutide/B12 order after the starter vial.” Every later vial “requires us to verify that you are under active medical care for this medication”, by uploading a current prescription, a chart note, a pharmacy label or a recent lab; “Existing Rx Peps Direct patients are auto-verified by email match and skip this step.”
  5. Billing and refunds, from the Refund Policy (“Peptide RX LLC, operating as Rx Peps Direct. Last updated: June 2026.”): “Rx Peps Direct charges a one-time $39 telehealth visit fee per order.” “If the request is received before provider review has started, Rx Peps Direct may approve a refund of the $39 visit fee.” “Once a licensed provider begins reviewing the patient’s intake, the $39 telehealth visit fee becomes nonrefundable because the medical review service has started.” “Rx Peps Direct cannot refund amounts paid directly to Optimal Balance Pharmacy.”
  6. Who does what, from the About page and FAQ: “The brand is operated by Peptide RX LLC and was founded in 2026.” “Rx Peps Direct and Optimal Balance Pharmacy operate as separate companies under a long-term supply agreement.” The footer: “All prescriptions are written by licensed NPs or PAs.” and “Rx Peps Direct serves patients in 29 US states (including the District of Columbia)”. The medical director, Dr. Jonathan Snipes, MD (NPI 1821250077), “practices through PWS-TX PLLC.” Two statements disagree with other pages: the terms (“Last updated: March 2026”) say prescriptions are written by “independently licensed nurse practitioners or physician assistants” while the About page says they “are employed by Rx Peps Direct”, and the terms' “Membership fees cover the prescription service” sits against the FAQ's “Rx Peps Direct does not sell memberships or recurring plans.” No postal address is published.
  7. The pharmacy, from /learn/optimal-balance-pharmacy and /about: “Optimal Balance Pharmacy is a 503A compounding pharmacy in Houston, Texas that fills every Rx Peps Direct prescription.” The About page lists “TSBP License #34829”. Every batch is tested by Eagle Analytical Services for sterility and endotoxin, with reports published on /quality. The FAQ: “Optimal Balance Pharmacy texts a secure payment link within 24 business hours of your approval email.” and “Your order does not ship until the pharmacy payment is complete.”
  8. No FDA enforcement action on record: FDA's warning-letter search on “Rx Peps Direct”, “rxpepsdirect”, “Peptide RX LLC”, “RxPepsDirect”, “Optimal Balance Pharmacy”, “PWS-TX PLLC”, returns no letter addressed to Rx Peps Direct, its operator, the named pharmacies or Rx Peps Direct. Search FDA's warning letters.
