The short version

In its favor

  • $199 buys one month of compounded semaglutide, no commitment
  • No membership fee on top
  • Clean FDA warning-letter record
  • Names its pharmacy: Manifest Pharmacy

Against it

  • The advertised $99/mo needs a 12-month commitment

What it is

Next Vial is an online clinic storefront from Next Vial LLC that sells compounded semaglutide and tirzepatide injections next to testosterone, hair-loss, ED, NAD+, sermorelin and a dozen other treatments. It takes no insurance and calls itself a marketplace connecting patients with independent providers; the FAQ names MD Integrations as its provider network and says every prescribing doctor holds a license in your home state.

You fill in a questionnaire and hear from a doctor within about 24 hours, though a handful of states call for a live chat or video consult first. A photo ID and a selfie are part of the intake. The service list covers every state except Alaska.

Prescriptions go to Manifest Pharmacy in Greer, South Carolina, the only pharmacy the FAQ names, and returns are addressed to “Next Vial LLC/Manifest Pharmacy” at the same street. Orders ship by rush no-contact delivery with no signature. The refund policy states that Next Vial doesn't currently ship refrigerated products, while its own GLP-1 FAQ says to keep these medications in the refrigerator between doses.

What it actually costs

Next Vial advertises $99. A month costs $199 because the advertised rate is the 12 months.

The gap

They advertise $99 a month. That is the 12 months, $1,188 on the card today. With no commitment it is $199 a month.

Next Vial prices by drug
DrugMonth to monthLowest rate on a longer plan
Compounded semaglutide$199$99 /mo, 12 months
Compounded tirzepatide$249$149 /mo, 12 months

Semaglutide is $199 for a single month, $169 a month on three months ($507 total), $149 on six ($894) and $99 on twelve ($1,188). For tirzepatide the ladder reads $249, then $229 ($687), $199 ($1,194) and $149 ($1,788). The page opens on the semaglutide twelve-month card, which is why $99 is the first figure you see.

Every plan includes the online consultation, the medication, syringes and alcohol pads, and ongoing provider support, and there's no monthly membership. Shipping is free on orders over $50, which every GLP-1 plan clears.

The cards print a total for each term without saying whether that total is charged at once. The FAQ says medication is billed when it ships, and that you'll be contacted before each refill is billed.

Lower on the same page, a heading reads “Doctor-Led Weight Loss Plans as Low as $249.97/Month”. No plan on the page costs $249.97; it's a few cents above tirzepatide's $249 month, the highest figure on the picker.

You can reorder or cancel from your account at any time before a shipment goes out. Once the charge goes through and the carrier has the box, there's no undoing it.

Who it's wrong for

Budgeting from the $99? That's semaglutide with a year committed and $1,188 printed as the total; a single month costs $199.

Living in Alaska rules you out, since it's the one state missing from Next Vial's service list.

Skip it if cold-chain shipping matters to you. The refund policy says refrigerated products aren't shipped right now, though the GLP-1 FAQ tells you to refrigerate between doses.

Not for you if you want Wegovy, Zepbound or another brand pen, because the GLP-1 page sells only the two compounded injections.

Once a package is handed to USPS, FedEx or UPS with your card charged, the sale is final, so a change of heart after shipping costs you the order.
