Compounded semaglutide is $199 a month at DrBrainRx and tirzepatide $299 at doses up to 7.5 mg, plus a required $25 monthly Care Access fee, so $224 and $324. The home page's $129 and $249 are six-month Premier rates.

What it is
DrBrainRx is the telehealth practice of Dr. Sara Siavoshi, a neurologist who lists triple board certification in neurology, headache medicine and obesity medicine. Its GLP-1 program, Skinny Brain, runs three tracks: compounded semaglutide and tirzepatide, brand-name Zepbound bought from Lilly Direct, and an insurance concierge that files prior authorizations for Wegovy, Ozempic, Mounjaro and Zepbound. Hair, sexual-health and NAD+ programs and a supplement shop sit alongside.
The operator is Siavoshi, Inc., doing business as DrBrainRX, and its storefront says it's based in San Diego. Visits are online and can be asynchronous. Prescribing is narrower than the marketing: the product pages say you must be physically in one of 21 listed states when the prescription is written (California, Florida, New York and Texas among them), though compounded medication can then ship to all 50.
Compounded vials come from U.S. pharmacies it describes as USP-certified but doesn't name, shipped cold-packed. DrBrainRx says it doesn't compound or ship anything itself, and processing after approval takes 2 to 7 business days.
What it actually costs
Advertised$129
Actually$224/mo
DrBrainRx advertises $129. A month costs $224 because the advertised rate is the 6-Month Premier (medication only).
The gap
They advertise $129 a month. That is the 6-Month Premier (medication only), $774 on the card today by our math, since the plan's total isn't printed. With no commitment it is $224 a month.

Medication prices. The required $25/mo membership is billed on top.
Two bills run side by side. Care Access is $25 a month on the compounded track and is the only automatic charge. Medication is paid separately: on the Monthly Flex plan, semaglutide is $199 per vial and tirzepatide $299, charged only when you ask for a refill.
Prepaying lowers the vial price. Semaglutide is $495 for three months ($165 a month) or $774 for six ($129 a month); tirzepatide is $807 or $1,494 ($269 and $249 a month). The six-month supply ships as two three-month boxes with a check-in between, and bundles can't be combined with the FAST50 code, which takes $50 off a new patient's first vial on the monthly plan.
DrBrainRx's own pages disagree on tirzepatide. The Skinny Brain page charges $299 for doses up to 7.5 mg and $399 above that, the home page tile says $249 “up to 15mg,” and the product page lists a single $299 monthly price for 2.5 mg through 15 mg. The home page's $129 and $249 match the product pages' six-month rates, not a single month.
On the Lilly Direct track you pay DrBrainRx $99 a month and pay Lilly for the Zepbound itself. The site quotes that medication price several ways ($299 or $349 for 2.5 mg, $399 to $499 for higher doses, and $650 when a refill comes more than 44 days later). A one-time $99 Insurance Check covers prior-authorization paperwork, and if coverage is denied the fee is credited toward compounded medication.
Who it's wrong for
Outside the 21 prescribing states? You'd need to be physically in one of them, such as Colorado, Michigan or North Carolina, when the prescription is written.
The home page's $129 semaglutide isn't a monthly price. It's the six-month Premier rate, $774 at once; a single month is $199 plus the $25 Care Access fee.
Skip it if you might want money back after starting. DrBrainRx keeps membership and consult fees once care has begun, and shipped vials aren't taken back.
Planning on tirzepatide above 7.5 mg? One DrBrainRx page puts it at $399 a month and another at $299, so get the figure confirmed in writing before paying.
