FormBlends review

GLP-1 telehealth · Prices read September 2026

Our verdict

At FormBlends, compounded semaglutide runs $165 a month on a Subscribe & Save plan you can cancel before any billing date, or $190 for a month bought outright, once a $99 first month is behind you. Tirzepatide opens at $149, then costs $214 subscribed or $239. Every dose costs the same, and the provider review, supplies and shipping are included.

The short version

In its favor

  • $165 buys one month of compounded semaglutide, no commitment
  • No membership fee on top
  • Clean FDA warning-letter record
  • Names its 503A pharmacies, including Greenwich RX (Greenwich Health & Wellness LLC)

Against it

  • The advertised $99/mo is its lowest rung; plan on $165

What it is

FormBlends is a GLP-1 storefront inside a sprawling peptide-education site: research reports, dosing calculators, a clinic directory and an iPhone tracking app surround the two things it actually sells to weight-loss patients, compounded semaglutide and tirzepatide. A two-minute assessment starts things off, and the prescribing and payment steps can continue on a sister checkout site that FormBlends links to.

Form Blends LLC, a Wyoming company at 30 N Gould St in Sheridan, says it isn't a medical practice and employs no clinicians. Prescriptions come from clinicians of Recess MD LLC, who must be licensed in your state, and medication is dispensed by independent 503A pharmacies. For GLP-1s it names Greenwich RX in Tomball, Texas, and The DrugStore on East State in Athens, Ohio.

Its reach is stated as a count, not a map. As of September 5, 2026, it had shipped to patients in 36 states and territories, going by its last 165 orders; in the other 16 you learn at assessment whether it can serve you, before anything is billed.

What it actually costs

FormBlends advertises $99. A month costs $165 because the advertised rate is the lowest rung of a dose ladder.

The gap

They advertise $99 a month. That is the lowest rung, not the price most people pay; the number a reader should plan around is $165.

FormBlends prices by drug
DrugMonth to monthLowest rate on a longer plan
Compounded semaglutide$165$99 /mo, 12 months
Compounded tirzepatide$214$179 /mo, 12 months

Semaglutide's first month is $99. After that a month costs $190 bought on its own, or $165 on Subscribe & Save, which renews monthly until you cancel. Tirzepatide follows the same pattern at $149, then $239 or $214. The provider review, medication, injection supplies and cold-chain shipping are included, and there's no membership or consultation fee. Lab work isn't covered.

Longer plans are charged in one payment. Semaglutide is $399 for three months, $714 for six and $1,188 for twelve, which FormBlends itself prices at $133, $119 and $99 a month. For tirzepatide the same terms cost $627 for three months, $1,194 for six and $2,148 for twelve ($209, $199 and $179 a month). None of them renews unless you pick Subscribe & Save, and a subscribed plan renews at $25 under the plan price.

Dose doesn't move the price; the pricing page says it's the same at 2.5 mg or 15 mg. Both drugs come co-formulated with vitamin B12.

The introductory month is said to apply to any plan, yet only the monthly and 12-month rows say what that means in dollars. The three- and six-month rows print their full prices, so the first charge on those plans isn't shown anywhere.

If the provider doesn't prescribe, the whole charge comes back, usually 5 to 10 business days later. Once a pharmacy dispenses, there's no refund, opened or not, though a damaged, warm or wrong shipment reported within 7 days with photos is replaced free. The GLP-1 program page also advertises a “30-Day Guarantee” for dissatisfied patients that the seven-rule refund policy never mentions.

Who it's wrong for

Reading $99 as the monthly price? It pays for semaglutide's first month and nothing after; from month two it's $165 subscribed or $190 without a subscription.

Sixteen places, among them Idaho, Louisiana and Oklahoma, are marked “Check availability at assessment”. None of FormBlends' last 165 orders shipped there, so eligibility is settled only at intake.

Not a fit if you'd rather talk than type. FormBlends runs no phone line; support is email, weekdays 9 to 6 Eastern, with callbacks by request.

Liraglutide isn't offered, and retatrutide and semaglutide drops exist here only as waitlists.

Need the dispensing pharmacy's name before paying? Support will tell you if you ask with your state; otherwise the label shows whether it was Greenwich RX in Texas or The DrugStore on East State in Ohio.

  2. The prices, from FormBlends' pricing page. Semaglutide: “First month $99, then $190 per month, or $165 per month with Subscribe & Save.” Table rows: “First month (introductory) $99 Applies once, to the first month of any plan”, “Monthly $190 $165 billed monthly”, “3 months $399 $133 per month $374 $125 per month, billed per 3-month period”, “6 months $714 $119 per month $689 $115 per month, billed per 6-month period” and “12 months $1,188 $99 per month $1,163 $97 per month, billed per 12-month period”. The same one-line summaries appear on each product page.
  3. How plans work, from the pricing page: “Every plan is a one-time upfront charge for the plan length. Nothing renews unless you choose Subscribe & Save. The introductory price applies to the first month of any plan: $99 for semaglutide, $149 for tirzepatide. Subscribe & Save renewals bill the plan price minus a flat $25 subscriber credit. The price does not change with dose: it is the same at 2.5 mg or 15 mg.” and “The 12-month plan is already $99 per month ($1,188 for 12 months), so the introductory first-month price does not change it.” Included: provider review, medication, injection supplies and cold-chain shipping; “Lab work is not included; shipping is” and “There is no membership fee”. “Liraglutide is not currently offered through FormBlends.” Retatrutide and “Oral Semaglutide Drops” are listed as “NOT YET AVAILABLE” with a waitlist.
  4. Billing and refunds, from the Refund and cancellation policy: “If the independent provider determines you are not eligible, or for any reason does not issue a prescription, the full charge is refunded to your original payment method.” “Once a pharmacy dispenses and ships your prescription, it cannot be returned, restocked or resold.” Damaged, spoiled or incorrect shipments reported “within 7 days of delivery” are replaced “at no charge”. To cancel, the policy says to email FormBlends' support email before that date. The /glp1 page separately lists “30-Day Guarantee If you are not satisfied with your experience, contact us within 30 days.” The terms apply Wyoming law and AAA arbitration with a 30-day opt-out.
  5. Who does what. Terms: “These Terms are between you and Form Blends LLC, a Wyoming limited liability company ("FormBlends", "we", "us"), 30 N Gould St, Ste R, Sheridan, WY 82801.” and “Certain services referenced on FormBlends' site may be completed through FormBlends' checkout site.” Pharmacy and quality: “Prescriptions are written by independent, state-licensed clinicians (MD, DO, NP or PA) of Recess MD LLC, a medical group separate from Form Blends LLC” and “The clinician must be licensed in your state.”
  6. Pharmacies, from the same page: “Greenwich RX Legal name: Greenwich Health & Wellness LLC Tomball, Texas DESIGNATION 503A compounding pharmacy DISPENSES GLP-1 therapies”, “The DrugStore on East State Athens, Ohio DESIGNATION 503A compounding pharmacy DISPENSES GLP-1 therapies”, and Chemistry Rx, Folcroft, Pennsylvania, a “503A compounding pharmacy and 503B outsourcing facility” that dispenses “Other compounded prescriptions”. “The dispensing pharmacy for any order is named on the prescription label and the shipping paperwork.” Shipping: “Orders ship after the provider approves and the pharmacy compounds, typically 3 to 7 business days.” and, from the pharmacy page, “Medication ships cold-chain with ice packs and you get tracking by email.”
  7. Reach and support. Where we serve: “As of September 5, 2026, FormBlends has dispensed compounded prescriptions to patients in 36 states and territories. That count comes from the last 165 shipped orders, not from a marketing map.” Marked “Check availability at assessment”: Alaska, the District of Columbia, Hawaii, Idaho, Louisiana, Maine, Mississippi, Nebraska, New Hampshire, New Mexico, North Dakota, Oklahoma, Rhode Island, Vermont, West Virginia and Wyoming. Contact: “There is no phone line; email is how we keep a written record of every request.” and “FormBlends' support email and FormBlends' support email are monitored Monday to Friday, 9:00 AM to 6:00 PM Eastern.”
  8. No FDA enforcement action on record: FDA's warning-letter search on “FormBlends”, “formblends”, “Form Blends LLC”, “FormScripts”, “Recess MD LLC”, “Greenwich RX”, “Greenwich Health & Wellness LLC”, “The DrugStore on East State”, “Chemistry Rx”, returns no letter addressed to FormBlends, its operator, the named pharmacies or FormBlends.
