$155 buys a month of compounded tirzepatide at the starter dose, on top of a $65 new-patient consult — all of it published on the open web with no intake required, which almost nobody here does.
Lavender Sky Health is the rarest thing in this category: a practice that publishes its prices on a public page and charges per visit instead of per month. "PAY PER CONSULT · No monthly subscriptions" is the first thing on the pricing block, and it means it.
It also names its pharmacy, which almost nobody here does, and asks which state you live in as the very first question of the intake.

Its Hallandale tirzepatide monthly plans read "Monthly Starter Dose – $155 total for one month supply · Recommended if new to treatment or for those on doses of 2.5mg/week or less", then "Monthly Mid Dose – $179", "Monthly Advanced Dose – $195" and "Monthly Advanced Plus Dose – $209", each "total for one month supply". ⚠ A month here is elastic: "Monthly Plans are typically dispensed as a single vial and are intended for use within five weeks." The multi-week tiers on the same page are quoted as totals with Lavender Sky's own per-month division beside them — "TIER 0 (20mg total) $219 total for 10 week supply · Monthly cost breakdown: $110", "TIER 1 (40mg total) $249 total for 8-10 week supply · Monthly cost breakdown: $99-$125 based on weekly dose", up to "TIER 4 (120mg total) $349 total for 8-10 week supply · Monthly cost breakdown: $139-$175". Those are ranges the company publishes, not divisions we performed, and we have left them as ranges rather than picking a number out of one. "Shipping and supplies are included." Separately, the practice offers Zepbound through LillyDirect at $299 to $449, on a "$25/month + the cost of medication" plan, which prices the program and leaves the drug at the manufacturer's own rate.
Consults are $65 for a new patient and $40 for a follow-up, each a flat fee. The pricing block's own headings are "PAY PER CONSULT · No monthly subscriptions", "UPFRONT PRICING · No hidden fees, ever", "DIRECT-PAY CARE · Insurance not required" and "FLEXIBLE CONSULTS · Electronic, phone, or video". Nothing about that arrangement requires you to start an intake to find out what you'll pay.
Medication is priced separately, per pharmacy and per dose, on a page that is more detailed than anything else in this roster. The Hallandale tirzepatide monthly plans run $155 at the starter dose, $179 mid, $195 advanced and $209 above 11mg a week, each described as a "total for one month supply". Multi-week tiers sit beside them as totals — $219 for ten weeks at 20mg, $249 for eight to ten weeks at 40mg, up to $349 at 120mg — and Lavender Sky does its own division for you, printing "Monthly cost breakdown: $99-$125 based on weekly dose" rather than a single flattering figure. Shipping and supplies are included.
The one thing to hold loosely is the word month. "Monthly Plans are typically dispensed as a single vial and are intended for use within five weeks", so $155 buys up to five weeks, not thirty days, and the tiers are quoted in weeks throughout. It's the most honest pricing page here and also the hardest to line up against a flat monthly subscription.
There's a brand route too — Zepbound through LillyDirect at $299 to $449 — on a plan described as "$25/month + the cost of medication". So the program fee and the drug are billed separately and both numbers are on the page, which is the opposite of how most of this industry handles brand medication.
One thing to know before you start: the consult fee is non-refundable once the consult has happened, and the intake requires you to upload a government photo ID. We stopped there. We won't upload a government ID or fabricate one, so nothing in this review describes what comes after that step.

Anyone who wants one number for everything. Here it's a consult fee plus a medication tier plus, on the brand route, a monthly program fee — three lines, all published, none of them bundled.
Anyone comparing this against a monthly subscription without doing the addition. A new patient's first bill is $65 plus whatever tier they're prescribed.
Anyone who won't upload a government photo ID. It's a required step in the intake.
Anyone who wants to cancel out of a consult they've already had. The fee is non-refundable at that point.
