A consultation fee, with a figure attached: 37 of 412

A charge for being seen by a prescriber, published somewhere on the company's own site. It is never folded into a monthly price anywhere on this site, because it is not monthly, and where a company publishes a range, it stays a range.

A consultation fee, with a figure attached
CompanyWhat it charges
AlloyA separate one-time charge that shows up only inside the checkout, never on a public page: “Initial Weight Care Consult”, “$99 one-time fee”, “Pay $99 for Your Initial Weight Care Consult”, with “Today's Payment $99.00”. You pay it before a prescription exists. $99 on the first visit, on top of whatever the medication costs.$99 per consultation
Amazon One MedicalThe weight-loss page advertises “Care from $39” and adds “Costs for medication and lab work are not included in the price of your care.” The general On-Demand Care page lists Direct Message Care from $29 and Video Care from $49; the weight-loss booking page prints no figure until a state is chosen. The fee is charged when a provider delivers a care summary. $39 on the first visit, on top of whatever the medication costs.$39 per consultation
Aspen HealthFrom the weight-loss FAQ: “A $20 medical consultation fee is presented during checkout as required by our clinical partner. It is paid to the provider for the clinical evaluation of your intake.” $20 on the first visit, on top of whatever the medication costs.$20 per consultation

Breeze MedsBreeze describes the $80 telehealth visit fee as conditional: charged only if the prescription is approved and you then choose not to proceed, and free if you go ahead. Its platform twin, Synergy Rx, describes the same $80 as charged "regardless of whether I receive medication". The two sites state opposite terms for one fee. $80 on the first visit, on top of whatever the medication costs.$80 per consultation
BrightmedsZepbound only. The products page shows “$100 Brightmeds doctor consult fee” on both the Zepbound pen and vial cards, while the compounded offer page says “There is no separate consultation or prescribing fee.” $100 on the first visit, on top of whatever the medication costs.$100 per consultation (brand tirzepatide only)
formidableMDThe weight-management page lists “$199 initial consultation” and “$99 follow up visit”; the terms of use say “$179 for initial appointment and $99 for each follow-up visit thereafter.” Patients on weight-loss medication follow up monthly for the first three months, then every three months. A $75 fee is kept on cancellations inside 48 hours. $199 on the first visit, on top of whatever the medication costs.$199 per consultation
Henry MedsA $30 medical consultation fee, part of what the refund policy calls Henry's “bundled billing structure”, collected for each provider evaluation that results in a prescription, per the refund policy and the terms of service. It is non-refundable once a prescription is issued, and refunds are also reduced by a processing fee whose size isn't published. $30 on the first visit, on top of whatever the medication costs.$30 per consultation

It covers the assessment, 69 baseline labs and an InBody scan, and there's no way through the funnel without it. $145 on the first visit, on top of whatever the medication costs.$145 per consultation
Kin MedsBrand-name track only. The branded-GLP-1 FAQ: “Step 1 — Apply & Screen ($49 one-time): Complete a health intake form and receive an at-home blood test kit.” and the fee “applies regardless of the outcome and is non-refundable as services are rendered during the screening process.” The pricing card beside it reads “Application & Screening $49 Complementary”. $49 on the first visit, on top of whatever the medication costs.$49 per consultation (brand semaglutide, oral semaglutide and brand tirzepatide only)
KnownwellFrom the FAQ's self-pay rates: “Weight management first visit: $299” and “Weight management follow-up visit: $149”, which “do not include the cost of prescriptions.” Self-pay patients pay when they book, and the home page promises monthly visits at the start. $299 on the first visit, on top of whatever the medication costs.$299 per consultation
Lavender Sky Health"New Patient Consult · For new patients starting with Lavender Sky Health · $65 per consult", with "Follow-Up Consult · For established Lavender Sky Health patients · $40 per consult" beside it, under the heading "Two consult types, one flat fee each". MadeMedKept only when a physician declines to prescribe. Under every GLP-1 plan card: “If not prescribed, you'll receive a full refund minus a $39 doctor visit fee.” An approved patient pays no separate visit fee; the plan price includes the physician consultation. $39 on the first visit, on top of whatever the medication costs.$39 per consultation

MD ExamThe FAQ prices it and makes it conditional: “Our consultation fee is $50. If you are not approved, the fee is 100% refundable.” All three product pages contradict that, listing “Free consultation” among their feature bullets. Both statements were live at the same time; the priced one is used here because a fee a reader might be charged is the one to budget for. $50 on the first visit, on top of whatever the medication costs.$50 per consultation
Medi-WeightlossOn the self-pay membership, the program “starts with a $199 New Patient Visit — which includes a medical assessment, lab work, and GLP-1 eligibility review”, after a free first assessment. Insured patients typically pay $0 to $50 per visit instead. $199 on the first visit, on top of whatever the medication costs.$199 per consultation

This is a visit fee, not a membership; Midi's pricing page states there are no membership fees. $250 on the first visit, on top of whatever the medication costs.$250 per consultation

Telezen MDThe refund policy, and only the refund policy, discloses a “$35 doctor visit fee”: it's deducted from any eligible refund once a provider has reviewed the case, including for a patient who isn't approved, and the 180-Day guarantee never refunds it. The plan prices don't show it as a separate line. $35 on the first visit, on top of whatever the medication costs.$35 per consultation
The Calibrate ClinicFrom the home page: “Pay only the $30 physician consultation deposit today. Try AgelessSection 8.2 of the terms (“Last Updated: August 20, 2026”): “All non-TRT services: a $40 clinical consultation fee is earned and non-refundable once the Provider consultation or clinical review begins or is completed.” It applies whatever the outcome, including when no prescription is written. Nowhere on the product pages is it clear whether the fee sits inside the listed price or comes on top. $40 on the first visit, on top of whatever the medication costs.$40 per consultation
Vita BellaOne-time, at sign-up, per the membership page FAQ: “A one time $99 consult fee applies at sign up, which covers your initial provider consultation and protocol development.” The terms add a $50 no-show fee for appointments missed without 24 hours' notice. $99 on the first visit, on top of whatever the medication costs.$99 per consultation

The weight-loss page also says “Book Your Free Consult” and calls the first hour “Completely free.” Consult fees are non-refundable once the visit is completed. $50 to $75 on the first visit. It stays a range here because they publish a range.$50 to $75 per consultation

What “none disclosed” means, precisely

It means that company's pricing page, its terms and its refund policy, as of the date printed on its review, disclose no membership requirement and no consultation fee. It does not mean no such charge exists, and it is not a promise about what lands on your card. Several of the fees above are disclosed only in a refund policy, a terms page or a checkout consent line rather than on the page that quotes the price; each row says where, in the company's own words. That is where these things live.

It also means the count moves. Every figure here is read out of the roster at build time, so the day a company's record changes, these tables change with it, and so does every price on the site that includes it.

How these charges are treated everywhere else on this site

Rows are ordered by company name, and every disclosed charge of this kind in the roster is here rather than a selection. Each quotation is close to the company's own wording and dated on its review. The method is here. Who pays us.

Companies where the published price is the whole bill are on the no-consult-fee board and the no-membership board. What a month costs with these included is on what will I actually pay; a year of it is on the twelve-month projection; one plan itemized is on the real cost calculator. Why a company's advertised figure and its bill are different numbers is on advertised against actual. The words in these tables are defined in the glossary.
